This is the title page and table of contents of the book.  It puts forth the four themes that will be covered in this book on the last plague of 1665 in London.  The first chapter covers the orders collected and published by Lord Mayor on the best ways to prevent the spread of disease.  The second section includes accounts of the plague made by Doctor Hodges.  The third section addresses means of prevention and cure.  The fourth includes weekly bills of mortality that display the prevalence and destructiveness of the disease from the Plague of 1592 to the Plague of 1665. The end of the book includes an account of the Plague of 1665 in Naples.  This section is most likely included as a comparison to the accounts and observations of Londoners.
